Making a referral

We are happy to accept referrals from care managers, placement teams or relatives.

As part of the referral process we will discuss your individual requirements and the various options we are able to offer. This will enable us to determine at an early stage if we are able to provide a service to meet your needs.

If we are able to offer a service, we will complete a comprehensive assessment of needs, with information provided from current services, family members and health professionals and where appropriate visits to home or existing placements. We are also happy to facilitate visits to Oakdown House or Carricks Brook at any stage of the process.

Once the assessment is complete, we will develop a support plan which includes details of the individuals planned support needs, including support to participate in activiites and access the community. The support plan is submitted to the funding authority, usually the local council, for negotiation and approval.

Once the cost of the support plan has been agreed, we will agree an individual transition plan leading up to an agreed moving date.

This process can take several months in the most complex cases, or a few days for emergency referrals.
